N1 - Funding Information: of females (59% vs. 41%; p = 0.016), secondary MDS (11% vs. 5%; p = 0.007), anemia (52% vs. 39%; p = 0.004), thrombocytopenia (41% vs. 28%; p = 0.002), neutropenia (49% vs. 40%; p = 0.037), transfusion dependency at diagnosis (50% vs. 26%; p < 0.001), blasts in blood (26% vs. 12%; p < 0.001), blasts in marrow >10% (27% vs. 9%; p < 0.001), LDH > 480 IU (26% vs. 16%; p = 0.01) and beta-2 microglobulin >3 mg/L (82% vs. 40%; p = 0.001). Additionally, marrow hypocellularity tended to be more frequent in patients with unsuccessful cytogenetics (15% vs. 9%; p = 0.09). A multivariate analysis using Cox proportional regression model and including patients with unsuccessful cytogenetics and with normal karyotype confirmed that unsuccessful cytogenetics was negatively associated with OS (p < 0.001). Other characteristics entering the model were transfusion dependency (p < 0.001) and bone marrow blasts (p < 0.001). In the study group of patients with unsuccessful cytogenetics the only variable independently associated with a worse OS in multivariate analysis was transfusion dependency (p < 0.001). Conclusions: These results demonstrate the independent prognostic impact of unsuccessful cytogenetics on OS in patients with MDS and that in them transfusion dependency has great prognostic relevance. Additional studies in this heterogeneous group of patients to identify the disease-or technical-related factors responsible for our observation are required.
